Description: Neoadjuvant, or preoperative chemotherapy for breast cancer is equally effective as postoperative chemotherapy in terms of locoregional and distant recurrence risk, but does improve the ability to perform breast-conserving surgery by an absolute amount of about 10%. Also, the degree of clinical and pathological response correlates with long term outcome (recurrence and survival), so this model has become in important research tool to obtain early estimates of the effectiveness of novel regimens and drugs and also to develop and refine prognostic and predictive classifiers based on imaging techniques (eg. MRI, PET scan) as well as tissue-based protein and genetic biomarkers. A prototypic study is currently being launched, called I SPY-2, that aims to test novel agents added to a standard chemotherapy backbone in biomarker-defined subsets with built in correlative MRI and tissue biopsy.
